Interactions

Alpha-Blockers

They reduce the adrenergic activity of the blood vessels and lead to a fall in blood pressure producing symptomatic hypotension. Thus you should avoid taking them along with Tadalafil which is also a vasodilator.

Amlodipine

If the patient takes tadalafil along with amlodipine the patient will experience a fall in blood pressure as Amlodipine is a calcium channel blocker and it relaxes and widens the blood vessels. So it will cause hypotension like tadalafil does.

Ritonavir and Other CYP3A4 Inhibitors

The human cytochrome CYP3A4 metabolizes tadalafil. The inhibitors of this cytochrome like the drug ritonavir will lead to a decrease in the metabolism of tadalafil in the body and the blood levels of this drug will increase. 

Alcohol

Alcohol is also a vasodilator and it will increase the vasodilatation action of Sildenafil. If you take too much of it your blood pressure will fall a lot and you can develop symptoms like dizziness, confusion, and heart palpitations. Some other severe problems may also develop.

Contraindication

Nitrates

Some cases have reported precipitous falls in blood pressure which has even led to myocardial ischemia and myocardial infarction due to the use of tadalafil in patients who were receiving organic nitrates.  This is so because nitrates increase the generation of cGMP, while sildenafil prevents its degradation by inhibiting PDE 5. Hearing Loss
